By submitting this certification of completion for this training module, I acknowledge and agree to the following as a condition of my return to in-person work:

1. I will follow and comply with the (CDC and KPC) guidelines for returning to in-person work, which includes but is not limited to: wearing a mask, practice social distancing, following hand hygiene protocols.  
2. If I am found not to be complying with the guidelines for retuning to in-person work, I may be subject to disciplinary action.
3. I agree that if I test positive or have symptoms for COVID-19, I will immediately notify Employee Health.
4. I agree that if I test positive or have symptoms for COVID-19, I will not report to the workplace and will immediately notify my supervisor of my absence.
5. I agree that I will cooperate with and participate in any contact tracing process used by KPC Health if requested.
